Eligibility

Who may align with an Atlas IPA Participant Provider?

To find out if you or someone you know is eligible for alignment, call (718) 814-6001.

  • Enrolled in Medicare Parts A and B
  • Medicare is the primary payer
  • Not covered under Medicare Advantage or another Medicare managed care plan
  • Resident of the United States
  • Resident of a county included in the Atlas IPA service area
  • For high needs population: one or more mobility-impairing conditions
  • For high needs population: at least one significant chronic or serious illness
